Service Agreement

Please review the agreement and terms of service below, and then complete the form at the bottom with your details and press the 'i agree' button to submit. I will not be able to work with you without a signed agreement, however if there are elements of the agreement you'd like to discuss, we can make space for that as part of an introductory online meeting.

Contracting is a standard part of coaching, mentoring, counselling and psychotherapy. It helps to set the foundation for our working relationship by clarifying expectations and processes.

​

Session days & times 

Once we agree to work together, your session days and times are reserved exclusively for you. Sessions are typically held weekly, unless we agree on a different schedule, such as bi-weekly or monthly. Days, times, and session lengths will be agreed upon prior to starting our work together, and can be adjusted if needed. All sessions start on the hour.

Online & Telephone working

Sessions will take place either online or over the phone. You will need to be in a private, safe space where you will not be disturbed. I will be in a private and confidential space. If in the event of technical difficulties when working online, I will reach out by a phone call. Online sessions will be offered using Zoom, unless we agree otherwise.

Endings

You are in total control of when you decide to end our work together. I offer single-session, short-term (for example, 6 sessions) and open-ended working, where no specific number of sessions have been agreed. These sessions are on an open, rolling basis, until informed of an ending by you. 

  

Notice of cancellations 

48 hours’ notice is required when you are not able to attend your session. Where 48 hours or more notice is provided, no charge applies for the session. If less than 48 hours notice is provided, sessions are still charged at the full agreed rate. Where possible, an alternative slot can be offered. Your appointments are reserved exclusively for you until you decide to end.

Missed sessions

Missed sessions are charged in full unless you have provided 48 hours’ notice. 

​

Late attendance 

The session will still finish at its usual time and will not run over. Please provide notice of lateness, if its safe and possible to do so. If no notice has been provided, 15 minutes into our session starting I will try to make contact using the phone number provided, this is to ensure you are safe and well.


If I have to cancel a session

In the unlikely event that I need to cancel our session, I will inform you as soon as possible and offer another slot at no extra charge.

Invoice/receipts  

Can be provided on request.

Block payment(s) 

Block payments can be made in advance for up to 4 sessions at a time rather than 48 hours before the session. When making a block payment, please inform me by email.

Block payment with cancelled Sessions 

If you've paid in advance for a block of sessions and cancel any one individual session with at least 48 hours' notice, the payment can be carried forward or refunded. Refunds are processed via bank transfer, usually within 10 business days and can be arranged by email.

​

3rd party payment

The payment agreement can differ if otherwise agreed in advance, for example where a 3rd party/employer is paying on your behalf. Additional arrangements will be put in place.

Concessions 

Concessions to fees can apply if agreed in advance based on your personal circumstances as part of the consultation.

Non-payment for session

Where payment for a session has not been made, the planned session will not go ahead, unless this has been agreed in advance. 

​

Contact

Communication between sessions can occur via text messages, phone calls, and emails. Typically I will respond within a short time, and please allow up to 48 hours for a response to any messages or calls outside of our sessions.


I may need to use the contact information you’ve provided to send you emails, texts, and other communications, which may be stored electronically. 


Due to the sensitive nature of our work, confidentiality is treated with the utmost importance, ensuring all communications are handled directly by me.


I am not able to offer crisis support. In the event of an emergency where I am not able to be contacted and/or I have not responded in the right way, it's recommended to dial 999 or 111.

Outside of sessions 

If we happen to see each other outside of sessions, I will not initially acknowledge you to maintain confidentiality and respect your privacy. You may be with someone who isn’t aware of our sessions. However, if you choose to acknowledge me, I will respond with a brief greeting.

Social media

I will not engage on any social media platforms using personal accounts. Any requests will be deleted, and messages will not be answered.

​

Reports

When written confirmation is requested by a third party on your behalf, it is kept to a minimum. I cannot provide detailed reports on sessions or offer opinions or diagnoses.

​

Client records

I am required by my professional membership body and insurance company to keep a written record of our work. I keep these are kept for a period of 7 years after the end of our work. After this period, such records are destroyed. Record information and notes are stored securely either in a locked filing cabinet, or electronically on a password protected computer to which only I (and my clinical executor) have access.

Clinical executor

I have appointed a clinical executor. In the unfortunate event I can no longer work with you, they will have access to your details and will get in touch with you.

​

Limits of confidentiality

The services I provide are confidential, meaning whatever you have shared remains between you and myself. However in in some circumstances*, the circle of confidentially may need to be widened. In these cases, and where possible/appropriate, i will discuss this with you in advance.

* for example:

  • where i'm concerned you pose a serious risk of harm to yourself and/or others

  • i become aware of involvement/knowledge of certain illegal activities including, but not limited to, money laundering & terrorism, where i am required by law to report. 

​

Self-referrals

If you have referred yourself, then no one apart from myself will know you have attended or what was discussed.

 

Clinical supervision 

I undertake regular clinical supervision to ensure good practice, any information regarding your sessions is anonymised to maintain your confidentiality.

Personal Details Form and Agreement to Contract

Submitting your details by this form is a formal agreement to engage with the services I provide. The contract is a living document, meaning it is subject to change. Any changes will be discussed with you. Some aspects of the contract may be verbal and evolve over time, making it impossible to list all eventualities and possibilities.
